About the role
The Community Paramedic acts as a visible and approachable first point of contact within the Community Hub, providing triage, assessment, short term intervention and referral through a no wrong door model. The role supports community members experiencing barriers to healthcare, reduces avoidable emergency department presentations, and contributes to the development of community paramedicine within a community health setting.
Key responsibilities include:
• Provide primary health care within scope of practice in community settings, with a focus on priority cohorts
• Deliver drop in clinics including triage, assessment, short term intervention and referral pathways
• Support implementation of funded programs including the Community Health Seniors Lifestyle Victoria Program and emerging initiatives
• Participate in design and delivery of new program offerings such as falls prevention and ageing well initiatives
• Work collaboratively within a multidisciplinary team including lived experience workers and interpreters
• Build trusting relationships with community members, particularly residents experiencing health inequities
• Contribute to culturally safe, inclusive and accessible service delivery
• Support the development and sustainability of the Community Paramedicine model at NRCH
• Monitor and reassess clients and maintain accurate clinical documentation using digital health systems
• Participate in program reporting, evaluation and continuous quality improvement activities
• Engage in community outreach, partnerships and promotional activities relevant to the Community Hub
About you
You are a community focused paramedic who is passionate about improving health equity and building meaningful connections with diverse communities.
You will bring:
• AHPRA registration and relevant qualification in Paramedicine
• Current Driver Licence
• Understanding of the Social Determinants of Health and their impact on health equity
• Strong communication skills and ability to build trusted relationships with community members
• Experience in community health or community paramedicine models (desirable)
