About the program
The Inner Melbourne Post Acute Care (IMPAC) Program forms part of the Health Independence Program delivered across St Vincent’s Health Melbourne and North Richmond Community Health. The program supports people to safely recuperate at home following discharge from hospital by providing practical and personal support.
About the role
The Community Care Worker provides direct support to clients in their homes, assisting with daily living activities where individuals are unable to manage independently due to illness, disability or frailty. This role supports client independence, wellbeing and safety through personal care, home care and community support services.
Key responsibilities
• Provide personal care including hygiene, dressing, grooming and mobility support
• Assist with meals, food preparation, eating and drinking
• Support clients with toileting and use of mobility aids
• Maintain safe and hygienic home environments including cleaning and laundry tasks
• Assist with shopping, errands and attending appointments
• Provide in home respite support for carers
• Deliver services in line with care plans, safety standards and program guidelines
• Maintain accurate records and provide updates to the care team
• Provide respectful, non judgemental and culturally responsive care
About you
You are a compassionate and reliable support worker who enjoys helping people maintain independence in their homes and community.
Key selection criteria
• Certificate III in Personal Care, Aged Care or Home and Community Care, or completion of first year Bachelor of Nursing
• Current First Aid and CPR certification
• Experience supporting older people or people with disability in personal care or home support
• Strong communication skills and ability to engage empathetically with diverse communities
• Current Victorian Driver Licence and confidence driving for work purposes
• Ability to work independently and manage time effectively
• Confidence using digital systems for communication and record keeping
