From primary health to outreach on the streets of North Richmond, we stand alongside people experiencing complex health and social inequities, with dignity, compassion and courage.
We are now seeking a Complex Care Coordinator to join our Alcohol and Other Drug Outreach and Engagement Services team.
About the role
This role provides holistic, trauma informed case management to people who use drugs in North Richmond. Working as part of a multidisciplinary team, you will deliver assertive outreach, clinical assessment and care coordination, while building strong relationships with clients, the Medically Supervised Injecting Room and the wider community.
The program operates seven days a week, and we are particularly interested in hearing from candidates who may be available to work some weekend shifts.
What you will be doing
Providing evidence based, harm reduction focused clinical care and case management
Delivering assertive outreach and community engagement in the North Richmond precinct
Completing holistic assessments and care plans within a shared caseload framework
Working closely with the MSIR and external services to coordinate care
Supporting overdose prevention, first aid response and naloxone education
Contributing to a safe, respectful and collaborative team culture
About you
You are a Division 1 Registered Nurse with current AHPRA registration and experience working with people who use drugs. You bring strong clinical judgement, excellent engagement skills and a commitment to trauma informed, culturally safe practice. Experience in outreach, AOD or complex care coordination is highly regarded.
