Outreach and Engagement Service | SACS Level 4

We are seeking a Mental Health Worker to join our Outreach and Engagement Service, a multidisciplinary team providing assertive outreach and complex care coordination to people who use drugs in North Richmond. This role supports people experiencing co occurring mental health and substance use issues through a trauma informed, harm reduction approach.

About the Role

• Provide assertive outreach, mental health assessment and care coordination

• Support clients to identify goals, safety needs and pathways to care

• Respond to crisis situations and provide secondary consultation to the team

• Build and maintain referral pathways to acute and non acute mental health services

• Work closely with peer workers, clinicians and community partners

About You

• Qualifications in Mental Health and or Dual Diagnosis

• Minimum three years experience in mental health, outreach, harm reduction or AOD

• Strong understanding of trauma informed practice and the social determinants of health

• Ability to work effectively in complex, community based environments
