Nurse Care Coordinator Position Description
About the program
The Seniors Lifestyle Victoria Program aims to reduce or reverse mild to moderate frailty in adults aged over 65 and reduce their risk of hospital presentations and admissions. The program provides coordinated, person centered care that addresses clinical needs alongside the social determinants of health to support sustainable health and wellbeing outcomes.
About the role
The Nurse Care Coordinator provides comprehensive nursing assessment, care coordination, and clinical management for clients entering the Seniors Lifestyle Victoria Program. The role acts as a central point of contact for clients, supporting their journey through the health and community system and ensuring safe, integrated, and high-quality care.
Key responsibilities include:
• Conduct comprehensive nursing assessments using a holistic, person centered approach
• Develop, implement, and review individualised care plans with clients
• Coordinate multidisciplinary care across internal teams and external providers
• Support clients to navigate health and social services, providing education and advocacy
• Monitor client progress and implement early interventions where required
• Facilitate safe discharge and transition planning
• Maintain accurate clinical documentation and support quality improvement activities
• Participate in case conferencing, care planning, and clinical review processes
• Provide clinical guidance and contribute to workforce capability development
• Contribute to continuous service improvement and best practice care delivery
About you
You are a compassionate and experienced Registered Nurse who is committed to person centred care and improving outcomes for older adults through coordinated, holistic health support.
Key selection criteria:
• Registered Nurse with current AHPRA registration
• Experience coordinating care for clients with complex health and psychosocial needs in a community or primary care setting
• Strong clinical assessment, care planning, and risk management skills
• Understanding of chronic disease management, health promotion, and social determinants of health
• Demonstrated ability to work effectively within multidisciplinary teams
• Excellent communication, documentation, and organisational skills
• Commitment to culturally safe and inclusive practice
• Experience using electronic client management systems
