About the role
We’re looking for a compassionate and strategic Integrated Care Manager to lead and coordinate the diverse health and social support services within our Integrated Care Zone, a dedicated drop in space within the Medically Supervised Injecting Room offering wraparound care such as Opioid Agonist Therapy (OAT), general practice (GP) clinics, blood-borne virus (BBV) testing, AOD (Alcohol and Other Drug) counselling, housing and legal support, and more.
In this role, you’ll:
- Lead a multidisciplinary team of clinicians and support staff working in the Integrated Care Zone
- Build and maintain partnerships with internal services and external agencies to improve access and integration
- Ensure the delivery of trauma-informed, harm reduction-based care to all service users
- Collaborate with GPs, nurse practitioners, and prescribers to coordinate high-quality medical care and maximise Medicare Benefits Schedule (MBS) billing
- Represent and advocate for the Integrated Care Zone in planning and leadership forums
- Support quality improvement initiatives and ensure compliance with all policies and procedures
About you:
- You have qualifications in nursing and are AHPRA registered
- You’re experienced in engaging with people who use drugs, particularly those who may have complex needs
- You bring strong leadership skills and know how to support and guide multidisciplinary teams
- You’re familiar with current harm reduction frameworks, mental health and AOD policy, and the broader health and social service landscape\
- You’re proactive, organised, and passionate about creating more connected, non-judgemental systems of care
