Permanent Part Time | 0.6–0.7 FTE | Grade 5 (HS5)

We are recruiting for an Service Coordinator to lead the Post Acute Care (IMPAC) program at North Richmond Community Health.

Post Acute Care supports people returning home following discharge from hospital or an Emergency Department, helping clients regain independence, access short-term support while other services are established, and reduce the risk of hospital readmission.

About the role

The Service Coordinator provides strategic and operational leadership to the IMPAC team and oversees the day-to-day delivery and development of the Post Acute Care program.

The role will:

• Lead, support and develop a multidisciplinary team

• Oversee high-quality, client-focused service delivery

• Manage program budgets, performance data and reporting

• Maintain strong clinical and corporate governance

• Support accreditation, audits, policies and continuous improvement

• Work closely with hospitals and service providers to support effective discharge planning

• Build strong relationships across the health and community care sector

About you

We are looking for someone with:

• An appropriate tertiary qualification

• Strong knowledge of the Victorian hospital and community care system

• At least five years’ recent experience in hospital and/or community health

• Experience leading and developing staff

• Experience managing budgets and provider contracts

• Strong problem-solving, relationship-building and communication skills

• A genuine commitment to community health and health equity
