About the role
Are you looking for a role where you can make a real difference in someone’s recovery journey—supporting them to regain independence and thrive at home after a hospital stay?
We’re looking for a compassionate and skilled Care Coordinator (Post Acute Care) to join our multidisciplinary team at North Richmond Community Health (NRCH). This is primarily an office-based role and sits within the Health Independence Program (HIP) and is delivered in partnership with St Vincent’s Health Melbourne.
In this role, you’ll be the first point of contact for clients transitioning from hospital to home, helping them navigate the service system and access the care they need. Whether you're a Registered Nurse or an Allied Health Clinician, your clinical knowledge and coordination skills will support clients to safely recover and maintain their wellbeing in the community.
Key responsibilities:
- Conduct intake, screening and client needs assessments
- Develop and review individualised care plans in collaboration with clients
- Coordinate a range of clinical and home-based supports tailored to each client’s needs
- Liaise with hospital staff, carers, community service providers and internal stakeholders.
- Advocate for clients and make referrals to additional services as needed.
- Participate in case conferences, discharge planning and family meetings.
- Maintain accurate electronic client records.
- Contribute to continuous quality improvement activities within HIP and NRCH.
- Participate actively in team meetings, training and organisational initiatives
Please note: This role can be offered to either a registered nurse or an allied health clinician.
Classification: Depending on the successful candidate’s qualifications, this role will be classified under either the Victorian Public Health Sector (Nurses and Midwives) Enterprise Agreement – YW15, or the Victorian Public Health Sector (Health and Allied Services, Managers and Administrative Workers) Enterprise Agreement – Allied Health Grade 2.

Key selection criteria:
Qualifications & Registrations:
- Relevant tertiary qualification as a Registered Nurse or Allied Health Clinician
- Current AHPRA registration
- Current Victorian Driver’s License (desired)
Experience:
- Minimum5 years' clinical experience in a health setting (hospital and/or community).
- Demonstrated experience working with clients with complex needs including mental health, homelessness, chronic illness, drug and alcohol, dementia.
- Sound knowledge of acute hospital discharge planning and community service systems.
- Experience working with culturally, linguistically and socially diverse communities.
Skills & Attributes:
- Strong care coordination and clinical reasoning skills.
- Excellent organisational and time management abilities.
- Capacity to problem-solve and work independently.
- High-level interpersonal and relationship-building skills.
- Proficient in Microsoft Office and electronic client record systems.
